Medical Records

RELEASE OF INFORMATION/MEDICAL RECORDS:

If you are needing medical records sent to or from our office, please click the corresponding link below to print out an “Authorization for Release of Information” form.

If you are coming to us from an outside provider, a form will need to be sent to the office where you were previously treated so that we may receive your records. Records are not automatically sent therefore they must be requested.

Directions

We are conveniently located in Franklin, Tennessee, right off exit 65 of Interstate 65, on the campus of Williamson Medical Center. Our offices are located at 3000 Edward Curd Lane. If you are unsure of which floor you need to go to for your appointment, please call us at (615) 791-2630.

 

From Nashville/Brentwood/North of Franklin: 

Take I-65 South to Exit 65. Upon exiting, turn left onto Highway 96/Murfreesboro Road. Turn left at the traffic light at Edward Curd Lane onto the Williamson Medical Center campus. Our facility will be on the left.

 

From Franklin/West of Franklin: 

Take Highway 96 East toward Interstate 65. After crossing over I-65, turn left at the first traffic light, Edward Curd Lane, and take the third left to the Bone and Joint Institute.

 

From South of Franklin:

 Take I-65 North to Exit 65. Upon exiting, turn right onto Highway 96. Turn left at the first traffic light, Edward Curd Lane, and take the third left to the Bone and Joint Institute.

 

From East of Franklin: 

Take Highway 96 West toward Franklin. Williamson Medical Center is on the right just east of I-65. Turn right at the Edward Curd Lane traffic light, and take the third left to the Bone and Joint Institute.

TSSAA Pre-Participation Medical Evaluation Form & Emergency Treatment/Parent’s Consent Form


Student-athletes experiencing concussion-like symptoms can see Dr. Richard Gibson for a return-to-play plan of care. Dr. Gibson specializes in the evaluation and treatment of concussions and related injuries. Throughout his training, he has served as team physician for multiple high school and college athletics departments, including providing coverage for Troy University, Samford University, Birmingham-Southern University and the SEC baseball tournament.
